What public records exist related to your onsite visit to 137 12th St, 
Apt 4A, on January 29 2015?

How would I go about getting copies of those records?

I would expect there are notes from your visit; also, I would hope that 
there was some sort of written communication, containing specific 
recommendations.

I would expect that there is a file related to 137 12th Street.
